We have many facets of communication. Patients can call us anytime or log onto the patient portal and send non-urgent messages.
To streamline the process and make your visit more efficient, please arrive on time or call as soon as possible if there is going to be a delay or if you need to reschedule. Please bring your patient information forms (which can be filled out online and printed prior to your visit), a list of all of your medications and your insurance cards. It is also your responsibility to know your co-pay and be prepared to pay at the time of service.
