Insurance Accepted

We accept most insurance plans. It is the patient’s responsibility to find out what their co-pay and deductible are as well as if a referral is needed by contacting their insurance companies about coverage benefits for allergy services. We expect co-pays and deductibles to be paid at the time of service. If you have Cofinity insurance, then please verify if there are any precertification requirements for allergy testing. If you have any type of Medicaid coverage, then your primary care physician must contact us to schedule an appointment.

Payments / Billing Arrangements

We accept the following payment methods:

  • Cash
  • Credit Card
  • Checks
  • Money Orders

If you have no insurance, then we ask that you pay $100 at the time of your first visit followed by monthly payments in order to keep your account active.

If you have problems paying your bill, please call our business manager. Please note that we make decisions on a case-by-case basis to determine how best to handle each situation.

If you have a large deductible or no coverage, please call ahead to make payment arrangements. We will come up with a plan that is best for you and the practice.
